With confidence (for once): this incision occurred while working on the hide.

Hides can reflect imperfections of various causes (scars, ducts, cysts, fibrosis, thinning, bullet holes, and so on), but an incision on the interior with no signs of an inflammatory process must be post mortem. Whether defects originating from the preparation of a hide – which this one perhaps can be – have to be accepted, I don’t know.
