I am not an apolsterer but i would cut two pieces the size of the seatplus an inch all around. Then cut a strip that will go around the four corners.
Put everything inside out and sew a seam all the way around to your finished dimentions. Turn the cover right side out and . Get your foam,put it in a plastic sack and stuff it in.
Oops forgot to add a zipper. So you should not forget that part.
